Reports/forms are available in two formats: PDF (portable disk format) and DOC (Microsoft Word). PDF format allows you to view or print the form using Adobe Acrobat Reader. If you don't already have the free Adobe Acrobat Reader, download it here. We recommend you download the item(s) you want now, and then visit Adobe's site to download the reader. Word format allows you to view/edit/save the form in your word processor. You may then print or e-mail the completed form. Completed reports may be emailed to Executive Offices.
